An independent review of the prison system has highlighted a raft of problems ranging from staffing shortages and poor infrastructure to failings in leadership and policy.

Announcing the findings in the Senate on Wednesday, Attorney General and Minister of Justice Senator Kim Wilkerson expressed confidence in the short term and long term plan to put things right.
